Can someone PM me an example of the 2017 fall bonus tag permit (you can black out or remove personal info if needed). I received mine back in the mail, but it has no identifiers on it, just looks blank. How do I "attach" it to a fish, I just received a piece of paper.

I feel i did something wrong, they sent something wrong or my wife messed up the mail (!) and can't reach anyone at Fish and Wildlife or the Nacote Creek office.

You should have received a thin metal tag, looks like a zip tie, as well as a blank log sheet.
Check the envelope, mabey it tore and the tag applied out

No metal tag seen. Just a slip with bonus info and log. It just looks like a blank log and I probably missed the tag in the envelope! What do I do! Can you PM me a pic and I can show my wife...

What can you do... look in the envelope. If no envelope you are out of luck and should call FGW for a replacement.
Can't figure out how to load a pic. I'll get the kids on it tomorrow.
It's basically a thin metal zip tie with # s on it.

This site is one of the most difficult sites to post a pic for me.
Mabey a server problem but most likely user error.
Anyhow it's a metal zip tie, you either got it or you didn't
Call the FGW and get a new one.
